Job Description:
A company is looking for a Pre-Visit Care Connector to
coordinate pre-visit assessments and provide care coordination via
telephonic communication.
Key Responsibilities
Conduct telephonic outreach and receive inbound calls from
members
Document demographic and medical history information accurately
Ensure members have access to their Primary Care Provider and
identify gaps in care
Required Qualifications
High School Diploma / GED or equivalent work experience
Minimum of 2 years of call center or telephonic customer service
experience, preferably in healthcare
2+ years of healthcare experience or experience in social
work/community outreach
1+ years of experience in collecting and documenting medical
history information
Proficient in Microsoft Word, Excel, and Outlook
