Job Description Company Description We are a home healthcare staffing agency that covers all of southern California, in which we need continuous additions to our team, as we are a growing company. Role Description This is a contract position for a Home Care Physical Therapist, located on-site in San Bernardino, CA. The role involves providing personalized physical therapy treatments to patients in their homes, collaborating with a multidisciplinary team to tailor rehabilitation plans, and monitoring patient progress. Responsibilities include evaluating patient needs, developing individualized treatment plans, delivering physical therapy services, and offering guidance on exercises for recovery and wellness. The therapist will ensure a high standard of patient care and maintain accurate documentation. Spanish Speaking is preferred but not required. Qualifications Proficiency in Physical Therapy practices, Rehabilitation techniques, and Home Care services Strong skills in delivering quality Patient Care and understanding of Medicine principles Licensure as a Physical Therapist in the state of California Excellent communication and interpersonal skills Ability to work independently and manage a flexible schedule Experience in home health or a similar setting is a plus Physical stamina to perform job-related tasks Commitment to maintaining patient confidentiality and delivering compassionate care
