Job Description:
A company is looking for a Clinical Pharmacist Patient Care.
Key Responsibilities
Conduct telephonic medication consultations that meet/exceed
production and quality metrics
Complete annual medication reviews and pharmacotherapy assessments
for patients
Deliver patient-centered consultations and document clinical
interactions
Required Qualifications
Active, unrestricted pharmacist license in the state of primary
residence
5+ years of pharmacy practice experience, including 2+ years in
Clinical Pharmacy Practice
1+ year of experience with Medicare STARs & HEDIS Measures
Proficiency in Microsoft Word, Excel, Outlook, and PowerPoint
Ability to manage multiple, concurrent projects in a dynamic
environment
