Job Description:
A company is looking for an OTR/L Care Manager.
Key Responsibilities:
Develop trusting relationships with members to support health
behavior changes
Assist in coordinating care and provide educational resources for
members
Encourage members to adopt habits for improved management of
chronic conditions
Required Qualifications, Training, and Education:
Bachelor's degree in Occupational Therapy (OT)
2-4 years of clinical experience with patient assessments and care
plans
2-4 years of experience supervising OTAs
Experience working with the Medicare population and geriatric,
frail populations
Experience utilizing Motivational Interviewing techniques
