The position is responsible for Medicare case management and leading the facility Interdisciplinary Team, care planning oversight, and a key clinical leader in the facility. Schedule is Mon-Fri with occasional weekend manager days.
We prefer an MDS Certified LVN or RN, and pay top dollar for talent. This position qualifies for a $1500 to $2000 sign-on incentive as well.
