the Admissions Department to ensure timely admissions with a high
degree of quality. Facilitates improved access to hospice services
through completing in-person admissions, education and role
modeling. Applies expert clinical judgment to ensure optimum
quality of patient care and services, while complying with
governmental and agency regulations. Supports agencies standards
and protocols through education and auditing.Job
Duties/Knowledge/Job Performance:
- Works on site in the Access Center to support the admission
process while working with internal teams/departments to enhance
referral-admit conversion.
- Completes in-person hospice admissions in any setting.
- Follows protocols to assure compliance with all federal, state,
and local health care regulations as they affect the provision of
admissions services.
- Demonstrates leadership in referral and assessment decisions
and communicates effectively with both external and internal
customers, including all staff of The Elizabeth Hospice (TEH).
- Monitors utilization of personnel to ensure acceptable/safe
productivity levels in conjunction with the Supervisors and
Managers.
- Helps to maintain agency census goals while nurturing
relationships within community referral base and collaborating with
Director of Sales & Marketing, and Chief Medical Officer
- Collaborates with the Admissions Manager to resolve quality
concerns.
- Reviews Admission notes to ensure adequate medical eligibility
and other required documentation which is then reflected in
clinical record. Seeks necessary pre-approvals per agency
policies.
- Provides clinical support and coordination for referral sources
including coordinating admissions from out of the area, as
needed.
- Ensures agency standards are followed for all
non-admissions.
- Compiles required reports such as staffing schedules,
appointments, changes etc. for the Admission department and
provides follow up in a timely manner with staff to ensure
compliance. Assists with projects as assigned.
- Assists with admissions and liaison duties such as discharge
planning, medication ordering, transport, etc.
- Keeps current with all department procedures and policies.
- Provides staff support including various clinical approvals and
verifications, as well as maintaining necessary electronic and hard
copy resources with Admission Manager approval.
- In collaboration with Supervisors, manages daily schedule.
Posts assignments to field staff in a timely manner.
- Other duties as needed and requested.
- DEGREE OF TRAVEL:
- Travel required for patient
- Occasional travel for educationalWork hours: Varied schedule -
Weekend rotation 10:30 -7:pm Weekend Commitment: Weekend rotation
Salary: $36.36 -$54.54/Hour
- Must have an Associates Degree, Bachelors Degree preferred, and
at least 4 years experience with 1-2 years preferred in acute care,
or equivalent combination of education and experience, and at least
2 years hospice experience required.
- Current CA RN licensure required.
- Prefer certification as a hospice palliative care nurse
(CHPN).
- Computer skills at minimum of Intermediate level
- Current California drivers license and proof of insurance
required.

Schedule:: Schedule is 5 shift 8 hour shifts per week to include
