Job Description:
SUMMARY:Provides clinical support to patients, families, and to
the Palliative Care team. Provides nursing care under the
supervision of the RN at the patient's place of residence. Assists
the RN and Intake Coordinator as needed with various tasks.
Communicates with both the internal and external customer. Displays
an ongoing commitment to service excellence to the internal and
external customers. Performs all duties in compliance with
regulatory guidelines. Responsible for carrying out the mission,
vision, and values of The Elizabeth Hospice.
JOB DUTIES/KNOWLEDGE/JOB PERFORMANCE:
- Facilitates communication between external and internal
customers and provides education on services provided by the
Elizabeth Palliative Care team.
- Performs licensed nurse skills as stated in the basic skills
checklist, policies and procedures, standards of care and critical
competencies (based on scope of practice) in order to provide
quality care and to meet regulatory requirements.
- Assists the RN with patient phone calls, pharmacy orders, IDG,
patient visits, communication with health plans, patients, and
community providers.
- Responsible for delivering services under the supervision of
and in collaboration with the RN.
- Electronically documents every visit and phone call according
to policy.
- Willing to work according to the daily schedule as assigned in
various locations (office, patient residence, hospital, and other
facilities) and adapt to changes to the schedule when
necessary.
- Portrays self-confidence with agency's reputation and
translates this image into a trusting relationship with patients,
families, and customers.
- Sets up telehealth system at patients' residence and provides
all telehealth training and assistance, including arranging for the
return of telehealth equipment. Also monitors and notifies the RN
or NP of any clinical alerts.
- Is cross trained to perform duties of the Intake Coordinator
when needed.
- Adheres to the E-way standards while performing job duties as
assigned.Location: Escondido -Work Hours Part Time in Escondido
Office. -3 - 8 hour shifts perweek 8:30am - 5pm -Salary: 27.68 -
$38.75 depending on experience
- One-two years of healthcare experience.
- Vocational specialized training.
- Current California LVN license.
- Proficiency in medical terminology preferred.
- Prefer experience with palliative care, hospice, home health,
managed care, Medicare and/or Medi-Cal.
- Demonstrates excellent customer service skills.
- Demonstrates ability to communicate effectively with patients,
physicians, community agencies and institutions.
- Computer skills (word processing), proficient typing ability,
scanner, copier.
- BLS Certified.
- Bilingual/ Bicultural (Spanish/English) a plus.
- Current driver's license and proof of insurance required.The
